What you can Expect:

Veterans Benefits Administration and Veterans Health Administration representatives will be available to assist you with:

• Providing information about VA benefits
• Reviewing and explaining decision rating letters
• Filing new claims for VA benefits
• Completing existing claims and/or appeals
• Adding any evidence you bring to your VA claims file
• Answering questions about health care eligibility
• Providing information about the VA ID card

What to Do Before the Event:

Follow the checklist below and gather any important information you’d like to bring with you. This will help ensure you make the most out of the event.

Review:

• Fact sheets for benefits you’d like to claim, such as, Disability Benefits Questionnaires

(DBQs), Fully Developed Claims (FDCs), Presumptive Conditions, and many more – OR – Go online to https://benefits.va.gov/BENEFITS/factsheets.asp to download fact sheets

Complete:

• One or more VA application forms, depending on the benefits you’re filing form. Find them online at www.va.gov/vaforms to complete before the event

o A VA staff member can also provide the forms you need and assist you with completing them

Bring with You:

• DD Form 214 (if available, not mandatory to have)

• Medical evidence to support your claim, including:

o Completed DBQs https://www.benefits.va.gov/compensation/dbq_disabilityexams.asp

• VA Form 21-4142, Authorization to Disclose Information to the Department of Veterans Affairs (VA
